Event Evaluation Form
Form to be completed by AAUW member holding a tabling or other public event. The form is in a writable PDF format that can be filled in on your computer and emailed or printed and mailed to the appropriate recipients.

Hold Harmless Forms for Events
Travel and recreation events where personal safety may be in question require each participant to sign a “Hold Harmless Form” once each AAUW year. Please complete this form Hold Harmless Form and send to the Interest Group Liaison at interest.aauw.poughkeepsie@gmail.com.

Photo Release Form for Minors
Photo Release Forms are required for minor children who are recognizable in the photos.
For photos where faces are not shown, no photo release is required. For joint activities with another organization, where that organization has an existing photo release, no additional release or copy is required. Likewise, if permission is granted via a registration form or other document signed by the parent/guardian, a separate release form is not necessary. Photo Release Forms are applicable to the event at which the photo was taken and are not to be applied to any other AAUW activity that the minor participates in. Thus, the release remains in effect in perpetuity for use of the photograph(s) associated with the event for which the parent/guardian approved that photographs be taken and used by AAUW.
